Job summary

Plum Creek Chicago is seeking compassionate and dependable CNAs to join our resident-centered care team. You will provide hands-on daily care, promote dignity and comfort, and collaborate with nurses to ensure individualized attention in a safe environment.

You will assist with activities of daily living, monitor vital signs, document care, and respond to call lights. We value teamwork, reliability, and compassionate communication with residents and colleagues.

Qualifications

• Current Certified Nursing Assistant (CNA) certification in the State of Illinois.
• High school diploma or equivalent preferred.
• Strong communication and interpersonal skills.
• Ability to work independently while contributing to a team environment.
• Dependable, compassionate, and professional approach to resident care.
• Basic computer and documentation skills.

Responsibilities

• Assist residents with activities of daily living (bathing, dressing, grooming, toileting, personal care).
• Assist residents with mobility, transfers, positioning, and ambulation as needed.
• Take and record vital signs and report changes to the nursing team.
• Respond promptly to resident call lights and requests for assistance.
• Assist residents during mealtimes and with feeding as needed.
• Observe residents and communicate changes in condition to the nursing team.
• Provide companionship, reassurance, and emotional support to residents.
• Maintain accurate and timely documentation of resident care.
• Help maintain clean, safe, and comfortable resident rooms and common areas.
• Follow infection control, safety, and resident care procedures.
• Work collaboratively with nurses, caregivers, and other team members to provide exceptional resident care.

Day Shift: 7:00 AM - 7:00 PM
Night Shift: 7:00 PM - 7:00 AM
Schedule: 3.5 days per week | Every other weekend required

About the Role

Plum Creek Chicago is seeking compassionate, dependable, and resident-focused Certified Nursing Assistants (CNAs) to join our team.

As a CNA, you will provide hands-on care and daily support to residents while promoting dignity, independence, comfort, and quality of life. You will work closely with our nursing team to ensure each resident receives attentive, individualized care in a safe and supportive environment.
