Responsibilities

1. Perform personal care needs and respond promptly to patient requests. Report any inability to perform tasks to the Case Manager.
2. Communicate positively with patients and families, maintaining confidentiality and ethical standards.
3. Ensure safety using equipment properly, including foot stools, side rails, and oxygen.
4. Provide personal care such as baths, oral hygiene, shampoos, and changing bed linens.
5. Assist with dressing, feeding, and mobility as instructed.
6. Monitor and record vital signs and observe skin integrity.
7. Report patient condition changes to the Case Manager.
8. Assist with medication reminders and light household tasks.
9. Maintain a clean and orderly living environment for the patient.
10. Adhere to documentation standards and participate in quality improvement activities.
11. Respect patient and family needs and environment.

Qualifications

• Meet state training requirements with at least 75 hours of training.
• Minimum six months of experience providing personal care.
• At least 18 years old, with good communication and organizational skills.
• Current CPR certification and understanding of hospice philosophy.
• Able to work independently and handle sensitive issues with tact and patience.
• Successfully complete a probationary period.
