Position Summary: The purpose of this position is to provide direct care to residents, under the supervision of a licensed nurse, in accordance with facility policies and procedures. The caregiver reports resident needs and concerns to a licensed nurse.

Essential Functions:
  1. Attend work as scheduled and follow company's call-off procedures.
  2. Follow established performance standards and perform duties according to nursing service policies and procedures.
  3. Identify resident problems and concerns and report them immediately to a charge nurse or licensed nurse.
  4. Complete assignments timely, completely, and accurately.
  5. Conduct resident rounds as assigned.
  6. Initiate corrective action as necessary and/or seek assistance of a licensed nurse.
  7. Attend all in-service classes as assigned and complete assignments.
  8. Document incidents and/or unusual problems according to established facility procedures.
  9. Review care plans and daily assignments and perform nursing care as outlined.
  10. Document in the nursing assistant notes the care and treatment provided to the resident and the resident's response or lack of response to care provided.
  11. Report changes in residents' conditions to Charge Nurse or Nurse Supervisor.
  12. Follow facility procedures for admission, discharge, and transfer of residents.
  13. Complete documentation accurately and timely, following facility policies and procedures.
  14. Listen to resident and family complaints and report problems to Charge Nurse.
  15. Ensure residents have call lights at hand and answer call lights promptly.
  16. Assist residents with personal and oral hygiene needs following facility policies and procedures.
  17. Assist residents with activities of daily living as documented in care plans.
  18. Ensure residents are given adequate assistance with meals, nourishment, and transportation to meal service areas.
  19. Check name bands on all assigned residents daily and report any issues to Charge Nurse.
  20. Observe infection control procedures.
  21. Be responsible for the safety of residents under supervision, including the use of safety devices and alarms.
  22. Protect the privacy of resident Protected Health Information.
  23. Maintain confidentiality and security of all resident and facility information.
Non-Essential Functions:
  1. Attend all required training, in-service, and staff meetings.
  2. Strive to maintain a safe working environment through accident prevention and safe practices.
  3. Maintain a positive and professional demeanor toward all associates, customers, vendors, and clients.
  4. Adhere to all policies and procedures of Spring Hills PASL, LLC.
  5. Perform other duties as assigned.
Experience and Education:
  • Current and valid Nurse Aide Certification.
  • A High School Diploma or GED; three to six months experience in LTC/Skilled care setting.
