Location: Menlo Park

The CNA (Certified Nursing Assistant) Caregiver reports to the Program Manager for the Geriatric Health Care for Homeless Veterans (Geriatric HCHV) program. This is a non-supervisory role responsible for providing specialized care and support to elderly individuals participating in the Geriatric HCHV program. Responsibilities include coordination of services, assisting with Activities of Daily Living (ADLs), and ensuring residents receive medical, social, and other supports/resources to live independently. The role involves working closely with the LVN (Licensed Vocational Nurse) Medical Case Manager, staff, residents, VA staff, healthcare providers, and community partners.

The CNA (Certified Nursing Assistant) Caregiver reports to the Program Manager for the Geriatric HCHV program. This non-supervisory role provides specialized care and support to elderly residents, including coordination of services, assistance with ADLs, and ensuring residents receive necessary medical and social supports to maintain independence. The position involves collaboration with LVN, staff, residents, VA, and community providers.

Job Duties

• Maintain a calm, friendly, and cooperative environment, promoting cleanliness and health.
• Assist with unit cleanliness and laundry to ensure a safe and comfortable environment.
• Provide transportation and schedule transportation as needed, including medical appointments and errands.
• Conduct appointment and medication reminders; monitor medication.
• Connect residents to VA and community resources to support independent living.
• Provide life skills education and assistance.
• Encourage social interaction and community participation.
• Collaborate with case management staff for coordinated care.
• Establish relationships with partner agencies to connect residents to services.
• Ensure accurate documentation in Electronic Health Records (EHR).
• Perform other duties as assigned.

Educational Requirements

Associate of Science (preferred), Bachelor’s degree in a related field (preferred)

Qualifications

• Certified Nursing Assistant License in California
• Basic Life Support Certification
• Minimum five years' experience as a caregiver
• At least one year healthcare experience
• Two years’ experience working with seniors or adults with disabilities
• Experience with mental health or substance use populations preferred
• Proficient in MS Office (Outlook, Excel, SharePoint, Teams)
• Ability to work independently with good judgment
• Clean DMV record

Physical Requirements

Ability to occasionally bend, stoop, reach, lift, and move items up to 20 pounds. Finger dexterity for computer use. Ability to assist with transfers of residents with limited mobility.

Work Environment

This in-person position reports to our location in Menlo Park, CA. It involves working in an office and residential environment. Remote work is not available.

Compensation & Benefits

$30-$34 per hour, depending on qualifications and location. Full-time benefits include health, dental, vision, life insurance, 401K match, holidays, and vacation. Part-time benefits may vary.
