Transitions At Home is hiring dependable CNAs/Caregivers in Wisconsin to support residents in an Independent Living Facility. You will provide personal care, transfer assistance, and companionship while following the care plan under RN supervision.

Qualifications

• Must meet Medicare Conditions of Participation for Home Health Aide training program and competency.
• Must have a current Wisconsin CNA License.
• Ability to carry out directions, read and write in English.
• Strong communication skills.
• Maturity and ability to deal effectively with the demands of the job.

Responsibilities

• Provide personal cares and transfer assistance.
• Perform household services and companionship.
• Report changes in condition to the RN Case Manager per the plan of care.
• Work under the supervision of the RN Clinical Manager.
