Job summary

Hacienda Healthcare in Phoenix, AZ is seeking compassionate CNAs and Caregivers (CCAs) to provide direct, person-centered care in our Intermediate Care Facility.

You will assist residents with ADLs, monitor health, follow ISPs, and participate in team trainings. This safety-sensitive role requires a background check, CPR/First Aid, and onboarding certification as needed.

Qualifications

• Must hold an active Arizona CNA certification in good standing.
• High school diploma or GED required or equivalent experience.
• Healthcare, long-term care, or developmental disability setting experience is a plus.

Responsibilities

• Assist residents with activities of daily living (ADLs) such as bathing, grooming, dressing, feeding, toileting, and mobility.
• Promote resident dignity, independence, and participation in activities.
• Document and report changes in resident condition to nursing staff.
• Follow infection control procedures and PPE usage, maintain a clean, safe environment.

What You'll Do

Direct Client Care

Assist residents with activities of daily living (ADLs), including:

• Bathing
• Grooming
• Dressing
• Feeding
• Toileting
• Repositioning and mobility assistance

• Promote resident dignity, choice, and independence.
• Support residents in social, recreational, and community activities.
• Escort and transport residents to appointments and activities as assigned.
• Participate in on‑campus and off‑campus resident activities.
• Ensure residents' personal belongings and equipment are maintained and ready for daily use.

Clinical & Support Responsibilities

• Measure and record vital signs (CNA responsibilities).
• Observe and report changes in resident condition to nursing staff.
• Assist with individualized care plans and active treatment programs.
• Follow all infection control and safety procedures, including proper use of PPE.
• Maintain a clean, safe, and sanitary environment.

Documentation & Compliance

• Complete required documentation accurately and timely.
• Follow Individual Support Plans (ISPs) and client‑specific programs.
• Maintain compliance with required certifications and training.
• Attend mandatory staff meetings and educational sessions.
• Adhere to all Hacienda Healthcare policies and procedures.

QualificationsFor CNA Applicants:

• Current Arizona Certified Nursing Assistant (CNA) certification in good standing.
• High school diploma, GED, or equivalent experience preferred.
• Previous experience in a healthcare, long‑term care, rehabilitation, or developmental disability setting is a plus.

QualificationsFor Caregiver / Client Care Advocate Applicants:

• High school diploma or GED preferred.
• Previous caregiving, direct care, healthcare, behavioral health, or customer service experience preferred but not required.
• Compassionate attitude and desire to support individuals with developmental and intellectual disabilities.

Additional Requirements:

• Ability to successfully complete required background screening.
• Fingerprint Clearance Card (or ability to obtain).
• Current CPR/First Aid certification or ability to obtain during onboarding.
• Ability to meet the physical demands of providing direct care.
• Strong communication and teamwork skills.
• Ability to work flexible schedules, including weekends and holidays as needed.
