Description

AW Health Care is seeking a dedicated and proactive CNA / Caregiver / Care Coordinator to join our team. In this patient care role, you will work closely with the Case Manager to ensure the well-being of our clients, both Medicaid and Private Duty, while supporting caregivers / CNAs and families. You will assist in a variety of tasks, including assessments, care plan creation, training, and documentation. The ideal candidate will have excellent communication skills, a passion for helping others, and the ability to manage multiple tasks effectively. The territory for this Care Coordinator is North, Central and South St Louis City and County.

Full Time Schedule: Monday - Friday, 7:00AM - 5:00PM (flexes), and every third weekend required.

CNA / Caregiver / Care Coordinator Responsibilities:

  • Assist Case Manager: Oversee and support community clients (Medicaid and PD) to ensure high-quality care and services.
  • Perform Initial Assessment Intakes: Assist in gathering relevant information during client intakes and contribute to the creation of Care Plans/ISPs.
  • Conduct Supervisory Visits: Assist with supervisory visits, meet and greets, on-the-job training (OJT), and client re-assessments.
  • Oversee New Hires and Cases: Provide oversight and guidance for new hires and new home care cases to ensure smooth integration and support.
  • Ensure Timely Documentation: Complete all required documentation within a 48-hour timeframe to ensure compliance and accuracy.
  • Community Engagement: Participate in local community events and presentations to promote services and raise awareness.
  • Develop Relationships: Build strong, trusting relationships with caregivers and families to ensure ongoing support and satisfaction.
  • On-Call Shifts: Participate in a rotating on-call schedule to address urgent client staffing needs.
  • Caregiver Education: Provide training for current caregivers on new skills and tasks to improve client care.
  • Assist Case Manager with Administrative Tasks: Support Case Managers in handling administrative tasks and ensuring smooth operations.
  • Caregiver Retention: Assist with caregiver retention efforts, including regular check-ins and team development to ensure a positive and supportive work environment.
  • Review and Address Issues: Regularly check in with Case Manager to review and resolve any concerns regarding care plans, clients, or caregivers.

Requirements

  • Must be at least 18 years of age.
  • Must have current driver's license, your own reliable transportation and auto insurance.
  • CNA / Certified Nursing Assistant Active Certification Required.
  • Knowledge of Medicaid and Private Duty services.
  • Must be able to read, write and document both physically and electronically, as well as follow directions.
  • Must have strong background in administrative duties, some scheduling knowledge preferred and training experience.
  • Ability to handle sensitive information and maintain confidentiality.
  • Must be able to work with clients that have pets or smoke.
  • Must be flexible with time and ability to adapt to changing priorities in the day.
  • To excel in this role, it is essential to showcase a profound understanding of growth and development principles across the entire lifespan. Additionally, you should have the capacity to analyze client data, accurately interpret pertinent information, and discern their unique age-related needs. This knowledge will empower you to deliver the care required, aligning with the agency's established policies and procedures.
