Job summary

Dignity Home Health and Hospice seeks a dependable CNA Coordinator to help manage and coordinate Certified Nursing Assistants serving patients across Utah County. This in-person role focuses on scheduling, communication with caregivers and families, and ensuring patient needs are met with compassionate, professional care.

You will organize CNA coverage, monitor documentation, and collaborate with physicians and interdisciplinary team members to uphold care plans and Medicare requirements.

Qualifications

• Must be organized and dependable with strong attention to detail.
• Experience coordinating schedules and caregiver communications is preferred.
• Ability to maintain patient confidentiality and professional ethics.

Responsibilities

• Organize and maintain CNA schedules to ensure patient coverage and service consistency.
• Coordinate visit coverage and assist with staffing changes as needed.
• Provide direct CNA support in the field when necessary within scope of practice.
• Maintain accurate patient-care documentation and ensure timely submissions.
• Communicate clearly with CNAs, patients, families, and other healthcare staff.
