Job summary

Focused Post Acute Care Partners in Baytown, TX seeks a Certified Nurse Aide to support residents with activities of daily living and promote independence and dignity. This full-time role emphasizes safety, compassionate care, and adherence to care plans.

You will perform bathing, grooming, feeding, mobility assistance and documentation in our resident-focused environment, while complying with Texas CNA requirements and record-keeping standards.

Qualifications

• Must hold current Nurse Aide certification in Texas.
• Must be in good standing on the Nurse Aide Registry and Misconduct Registry.
• Ability to understand, remember and carry out verbal and written English instructions.
• Ability to communicate clearly and hear well enough to interact with residents.

Responsibilities

• Ensure resident safety.
• Bathe, shower, shampoo, shave, comb hair, dress residents, nail care.
• Follow the assignment schedule set by the charge nurse.
• Lift, move and transfer residents as required.
• Answer call lights promptly.
• Assist or feed residents.
• Maintain cleanliness and toileting care for residents.
• Provide supportive and safe environments for residents.
• Utilize electronic medical records charting (PointClickCare).

QUALIFICATIONS

• High school diploma or equivalent (GED)
• Must have current Nurse Aide certification in the state of Texas
• Must be in good standing on the Nurse Aide Registry in the state of Texas and Misconduct Registry
• Ability to understand, remember and carry out verbal and written instructions in English
• Ability to speak clearly and hear well enough to communicate with residents

ESSENTIAL FUNCTIONS

• To ensure resident safety.
• Bathe, shower, shampoo, shave, comb hair, dress appropriately, nail care of any residents assigned
• Follow resident assignment schedule as made out by the charge nurse
• Lift, move, and transfer residents as required
• Answer call lights in a timely manner
• Assist or feed residents
• Keep resident clean and dry, toileting or providing incontinent care
• Provide supportive, protective, and safe environments for residentsOther duties as assigned
• Utilize electronic medical records charting; point click care

PHYSICAL REQUIREMENTS

Have the ability to safely perform movements such as pushing, pulling, lifting, bending, kneeling, reaching and lifting up to 50 pounds with or without reasonable accommodations
