Job summary

Robin Hill Farm, an affiliate of Rehab Without Walls, offers a CNA role in a home-like residential neuro rehabilitation setting. You will provide direct personal care, assist with daily living activities, and help residents build independence as part of a interdisciplinary team dedicated to dignity and growth.

The position includes medication observation, transportation support, meal preparation, and participation in team meetings and ongoing professional development to ensure quality of life

Qualifications

• Must be at least 18 years old.
• Current CNA certification or Home Health Aide certification per state requirements.
• Valid driver’s license with satisfactory driving record.

Responsibilities

• Provide direct personal care (bathing, dressing, grooming, toileting, feeding, ambulation).
• Monitor residents’ health and report changes to clinical staff.
• Assist with medication observation as directed by care plans.
• Support residents in developing daily living skills and independence.
• Escort residents to appointments, outings, and activities.
• Document care provided and progress accurately and timely.

As a Certified Nursing Assistant (CNA), you'll do more than provide direct care—you'll help residents build independence, confidence, and life skills while supporting them in their recovery journey. This unique role combines traditional CNA responsibilities with opportunities to assist with medication administration, transportation, meal preparation, community outings, and daily household activities. Working alongside an interdisciplinary team, you'll build lasting relationships with residents and help create a supportive environment focused on dignity, growth, and quality of life.

Responsibilities

• Provide direct personal care assistance, including bathing, dressing, grooming, toileting, feeding, and ambulation.
• Monitor residents' physical condition and report changes in health status to the appropriate clinical staff.
• Assist with medication observation and medical monitoring as directed by care plans and organizational policies.
• Support residents in developing and maintaining daily living skills and self-help activities.
• Encourage independence while providing appropriate supervision and assistance.
• Foster positive relationships and healthy interactions among residents and their housemates.
• Ensure the safety and well-being of residents at all times and maintain a safe living environment.
• Assist residents with recreational, social, and community-based activities.
• Accompany residents to scheduled appointments, community outings, and other approved activities.
• Document care provided, resident progress, and significant observations accurately and timely.
• Communicate effectively with residents, families, healthcare providers, and team members.
• Follow individualized care plans and support strategies.
• Maintain compliance with all agency policies, procedures, and regulatory requirements.
• Participate in team meetings, training programs, and ongoing professional development activities.
• Perform other duties as assigned to support resident care and program operations.

Qualifications

Required

• Must be at least 18 years of age.
• Current Certified Nursing Assistant (CNA) certification, or Home Health Aide certification if applicable, in accordance with state requirements.
• Valid driver's license with a satisfactory driving record as defined by agency policy.
• Completion of a recognized nursing assistant or home care aide training program.
• Minimum of six (6) months of recent experience providing direct care or related support services preferred.
• Ability to communicate effectively both verbally and in writing.
• Demonstrated compassion, patience, and commitment to person-centered care.

Preferred

• Experience working with individuals recovering from neurological injuries, brain injuries, behavioral health conditions, or other complex rehabilitation needs.
• Experience in residential care, long-term care, rehabilitation, or community-based support settings.

Salary Range

USD $21.88 - $23.10 / Hour
