Responsibilities

• Assist patients with daily living activities (bathing, dressing, grooming).
• Provide mobility and ambulation assistance, including transfers and walking with residents.
• Assist with personal care (toileting, incontinence care, feeding).
• Monitor and report changes in patient condition, vital signs, and behavior.
• Maintain accurate records and documentation of care and treatments.
• Collaborate with interdisciplinary care teams to develop care plans.
• Provide support and companionship to residents to promote community.
