Job summary

Swan Lake Nursing & Rehabilitation in Patchogue, New York, is seeking a Certified Nursing Assistant (CNA) for a per diem position. You will work under the supervision of registered nurses and licensed practical nurses to provide high-quality care to residents.

Responsibilities include assisting with daily living activities, feeding, mobility, vital signs, and ensuring a safe, clean environment. NY CNA certification and a HS diploma are required; 1 year of CNA experience is preferred.

Qualifications

• Current Certified Nursing Assistant certification in the State of New York
• High School diploma or equivalent required
• 1 year of experience as a CNA preferred
• Ability to lift, push, and pull equipment and supplies up to 50 pounds
• Ability to work in a fast-paced environment and adapt to changing situations

Responsibilities

• Perform selected resident care activities, including assistance with daily living tasks, bathing, grooming, and dressing
• Assist with feeding, toileting, and ambulation as needed
• Provide vital sign monitoring, including temperature, pulse, and blood pressure
• Assist with transportation of residents between areas of the facility
• Record intake, output, BM, vital signs, toileting, range of motion, ambulation, nutrition, ADL care
• Assist with preparing residents for examinations and treatments
• Demonstrate appropriate restraint application and release
• Maintain a clean and safe environment for residents and staff

Job Summary:

Responsibilities:

• Perform selected resident care activities, including assistance with daily living tasks, bathing, grooming, and dressing
• Assist with feeding, toileting, and ambulation as needed

• Collect and transport specimens as needed
• Respond to call bells and alarms in a timely manner
• Ensure personal alarms are in place and functioning according to individual residents' plans of care
• Communicate any changes in residents' conditions to the nurse
• Record intake, output, BM, vital signs, toileting, range of motion, ambulation, nutrition, ADL care, security device bracelets, showers/bathing, and skin checks as per facility policy
• Assist with preparing residents for examinations and treatments
• Demonstrate appropriate restraint application and release
• Maintain a clean and safe environment for residents and staff

Requirements:

• Current Certified Nursing Assistant certification in the State of New York
• High School diploma or equivalent required
• 1 year of experience as a CNA preferred
