Job summary

JourneyCare Home Health in Chicago, IL is seeking a dedicated CNA to provide personal care and help with ADLs for patients in their homes. This field-based role offers an in-home care setting with a Monday–Friday, 8am–5pm schedule and competitive hourly pay.

You will work closely with registered nurses, perform baths, grooming, meal support, mobility assistance, medication reminders, and ensure safety and dignity.

Qualifications

• One-year experience in hospital or nursing home setting.
• Completed recognized nurse assistant training program desired.
• At least 18 years of age and is a high school graduate or equivalent.
• Ability to read and follow written instructions and document care given.
• CPR certification, if required.
• Must have reliable transportation; own vehicle or access to public transport.

Responsibilities

• Provide hands-on personal care including baths, back rubs, oral hygiene, shampoos, changing bed linen, dressing and undressing, skin care, toileting, and maintaining patient living areas.
• Plan and prepare nutritious meals.
• Assist in feeding the patient, if necessary.
• Transport client to doctor’s appointments, store, and other errands.
• Assist in ambulation and exercise per plan of care.
• Assist patient with self-administration of medication.
• Perform patient laundry as appropriate.
• Ensure safety and proper use of equipment.
• Report patient condition and changes to the assigned nurse.
• Maintain confidentiality and professional conduct.
• Maintain regular attendance and flexibility to meet department needs.
• Follow Medicare, Medicaid, and HIPAA regulations.
• Perform other duties as assigned.

Hourly Pay Range: $19 - $26

Schedule: Monday-Friday, 8am-5pm

What You’ll Do:

• Providing hands-on personal care including baths, back rubs, oral hygiene, shampoos, changing bed linen, assisting patients with dressing and undressing, skin care to prevent breakdown, assisting the patient with toileting activities, and keeping patient’s living area clean and orderly, as appropriate.


• Planning and preparing nutritious meals.


• Assisting in feeding the patient, if necessary.


• Transport client to doctor’s office, grocery store and other essential errands.


• Assisting in ambulation and exercise according to the plan of care.


• Assisting patient in the self-administration of medication.


• Doing patient’s laundry, as appropriate.


• Meeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).


• Reporting on patient’s condition and significant changes to the assigned nurse.


• Maintain a high degree of confidentiality at all times due to access to sensitive information.


• Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the department.


• Follow all Medicare, Medicaid, and HIPAA regulations and requirements.


• Abide by all regulations, policies, procedures and standards.


• Perform other duties as assigned.
