Job summary

Responsibilities

• Deliver sustained quality and outcomes improvements across value-based programs.
• Lead physician alignment and collaboration with health plan partners.
• Oversee care models, access, and care coordination across populations.
• Establish performance expectations and drive data-informed improvements.
• Maintain clinical credibility through active practice and leadership visibility.
• Collaborate with multidisciplinary teams including RN Case Managers and Care Coordinators.
