Job summary

Continuum of Care in New Haven, CT seeks a Cluster Program Manager to oversee day-to-day goals for A Common Bond and Brownell programs. You will plan, supervise staff, manage budgets, ensure client-centered services, and coordinate with partners to deliver comprehensive care.

Responsibilities include admissions/discharges, on-call coverage, staff training, and maintaining compliant records. A valid CT driver’s license and CPR/First Aid certification are required or pursued within 90 days.

Qualifications

  • Bachelor's degree preferred with 1 year of supervisory experience; or 5 years in the field with 2–3 years of supervisory experience.
  • Experience with mental health, addiction and/or intellectual disabilities required.
  • Valid Connecticut driver's license and reliable transportation for client transport as needed.
  • First Aid/CPR certification or willingness to obtain within 90 days of hire.
  • Proficient with MS Office and electronic record keeping.

Responsibilities

  • Oversee day-to-day program goals and service delivery for assigned sites.
  • Manage budget and allocate resources in coordination with the Director.
  • Assign caseloads, staff duties and scheduling; ensure staff/client rapport.
  • Coordinate with agencies and community services for client care.
  • Provide supervision, staff development and performance evaluations.
  • Respond to crises and provide on-call coverage as needed.
  • Ensure confidentiality and HIPAA compliance in documentation and reporting.

Position: Cluster Program Manager

Program: A Common Bond and Brownell

Scope of Service: Responsible to operate and oversee the day‑to‑day program goals and objectives for the effective, efficient, and satisfactory delivery of service to persons served. Provides comprehensive planning and support for the coordination, implementation, scheduling, and evaluation of all program responsibilities. Provides assessment and decision‑making skills to manage the program and any associated issues on a daily basis. Provides professional development to staff, interns, and volunteers. Reports to the Program Director and is involved in on‑call responsibilities. Performs other duties as assigned.

ESSENTIAL JOB FUNCTIONS
Program Management
  • Works with Program Director to ensure compliance and delivery of program goals and objectives.
  • Facilitates and monitors effective day‑today delivery of services.
  • Provides assessment and decision‑making skills needed to manage the program and any associated issues on a daily basis.
  • Manages assigned budget, monitoring costs and allocation of resources in collaboration with Director.
  • Assigns caseloads, program duties, and determines scheduling of staff.
  • Establishes and ensures rapport and positive working relationships with clients while maintaining professional ethics and boundaries for all team members.
  • Ensures client needs are continuously met included but not limited to: daily living skills, overall well‑being, referrals, obtaining and maintaining entitlements, medical and psychiatric care, transportation, housing, wellness/nutrition, legal services, education and employment.
  • Monitors effective engagement between staff, clients, volunteers, and identified program supports.
  • Ensures that services are person‑centered and responsive to individual needs.
  • Schedules, attends and participates in agency, team, and interdisciplinary meetings.
  • Facilitates psychoeducational groups as indicated to support client growth and life‑skill development.
  • Ensures company vehicles are in good working condition and completes periodic maintenance inspections work orders as necessary, maintaining vehicle cleanliness and operation.
  • Maintains property to ensure building/office is welcoming upon entrance and works with team to promote a clean, safe, and organized work environment for both staff and clients. Processes timely work orders with Facilities as necessary.
  • Prepares and ensures compliance with Quality and Chart audits, Billing audits.
  • Maintains benchmark guidelines and develops action plans as identified.
  • Ensures continuity of care for delivery of services.
Clinical Services and Supervision
  • Supervises program admissions, planning, and discharge processes.
  • Plans and facilitates staff and house/community meetings.
  • Facilitates coordination with other agencies and community services ensuring appropriate client care.
  • Monitors documentation of services and the collection of data for contracted/agency reporting.
  • Responds to crisis situations and provides appropriate interventions, de‑escalation, and debriefing.
  • Provides rotating on‑call coverage.
  • Works with Program Director in the recruitment, selection, and hiring of staff.
  • Provides and documents supervision and performance evaluations consistent with agency policy.
  • Provides new staff with program orientation and ongoing professional development and training.
  • Addresses concerns with staff or other professionals constructively and in a timely fashion.
Quality Assurance
  • Adheres to policies and practices as outlined in staff handbook and program operations manual.
  • Consistently works in a respectful, constructive, and cooperative manner.
  • Upholds all Health Insurance Portability and Accountability Act (HIPAA) regulations and secures all necessary releases of information for outside communication.
  • Models culturally competent behavior interacting with clients, providers, and fellow staff.
  • Maintains accurate electronic records and case files as assigned and in accordance with programming, contractual, licensing, and accreditation requirements.
  • Adheres to the standards of the Council on the Accreditation of Rehabilitation
QUALIFICATIONS
Single Site Program Manager
  • Responsible for oversight and management of one assigned program.
  • Bachelor's degree preferred with a minimum of one (1) year of supervisory or managerial experience. Candidates with a high school diploma or equivalent may be considered if they possess a minimum of five (5) years of experience in the field and (2) two to (3) years of supervisory experience. Current agency employees may be considered with a minimum of two (2) consecutive years of supervisory experience within the agency.
  • Prior experience working with people with mental illness, addiction disorders, and/or intellectual disabilities is required.
  • Valid Connecticut Driver’s license is required. Scattered site and non24 hr. program positions require the employee to maintain reliable transportation and be willing to transport clients in personal vehicles.
  • First Aid/CPR certification or willingness to obtain certification within 90 days of hire. Employee is responsible to maintain ongoing certification.
  • Proficient computer skills required, including Microsoft Office and electronic record keeping.
