Job description

Under the supervision of the CLOIP Service Coordinator Supervisor and within federal, state and local guidelines, provides quality case management services to individuals with Intellectual and Developmental Disabilities within Denton County. Performs routine case management work. Work involves developing and maintaining long-term contact with individuals, individuals' families, and service providers for medical, social, educational, and related service needs.

The Service Coordinator is responsible for a caseload of approximately 80 individuals and the maintenance of current and accurate written information. The Service Coordinator will maintain good working relationships with individuals served, their families, the designated Local IDD Authority, state supported living center staff, and community providers. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.

Education, Training, and Experience

  • Education Graduation from an accredited four-year college or university with major coursework in social, behavioral, human services, or a related field required.
  • One-year MHMR experience is preferred. Experience and education may be substituted for one another.

Registration, Certification, Licensure, and Other Qualifications

  • Must have and maintain a background and criminal history free from any disqualifying offenses as outlined by the Texas Administrative Code (TAC) and the Health and Human Services Commission (HHSC).
  • Must possess and maintain a valid driver's license and automobile insurance.
  • Individuals with an out-of-state driver's license must be able to obtain a driver's license in the state of Texas within thirty (30) days.
  • Successful completion of all position-specific training within thirty (30) days of employment is required.

Knowledge, Skills, and Abilities

  • Knowledge of community resources, of case management delivery systems, and of program policies and procedures.
  • Ability to assess individuals' needs, to coordinate individuals' services, to communicate effectively, and to train others.
  • Skill in the operation of computers.
  • Ability to collect and analyze data, to prepare and maintain records.
  • Ability to communicate effectively.
  • Ability to display regular and reliable (in-person, if required) attendance.
  • Understanding and practice of Trauma Informed Care principles.

The following list outlines key responsibilities for the position; however, it is not exhaustive and does not encompass all responsibilities. Additional duties may be assigned as needed.

Provide and explain ?Explanation of IDD Services and Supports? and ?Long Term Services and Supports? to all adult residents of DSSLC and / or their LAR at least one time per year, unless the individual and / or LAR refuses the information. This refusal must be documented.

Communicates effectively with less than 5% complaints about presentation of information.

Provide and explain other informational and educational material approved by DADS that will provide a more complete explanation of specific types of services, unless the individual and / or LAR refuses the information. This refusal must be documented.

Search for additional information to provide to individuals, LARs, and DSSLC staff.

Complete the community living options process and provide a written report of the CLOIP process to the SSLC no later than 14 calendar days prior to the individual's SSLC annual planning meeting. Mail worksheet to designated LIDDA.

Has zero (0) worksheets submitted over 14 days prior to annual planning meeting.

Attend the SSLC annual planning meeting in person or by teleconference 100% of the time or arrange with the supervisor for an alternate LIDDA staff to attend, unless the LAR or individual has specifically requested the LIDDA not participate.

Attends 100% of DSSLC annual planning meetings when permitted by individual / LAR.

Complete all required documentation timely and as required in accordance with Center policies and procedures as well as DADS contract and TAC requirements. Maintain at least 95% compliance on all documentation.

Comply with contract performance measures.

Documentation is completed for all individuals served within allotted time frames with 95% compliance.

Maintain good working relationships with individuals served, their families, the designated LIDDA, State Supported Living Center staff, and community providers.

Communicates effectively with less than 5% complaints.

Perform additional duties as deemed necessary by department supervisor within designated timeframes.

Collaborates with supervisor on additional duties.

Full Time : Non- Exempt

Monday through Friday : 8am-5pm

Pay Range : $19.00-$20.00

Full-time employees are eligible for full medical, dental and vision benefits. Part time employees are eligible for full medical and dental benefits. Incentives are available if employee chooses to participate in Center's Wellness Program.

The Center covers 100% of cost of HMO if electing employee coverage only.

Full-time employees earn up to 120 hours of PTO during first year of employment.

Part-time employees earn up to 60 hours of PTO during first year of employment.

  • PTO increases with tenure level.
  • The Center gives 5 personal days to full-time employees and 1 personal day to part-time employees to use at their discretion.
  • Full time employees are enrolled into 401a retirement plan at 90 days of service.

The Center contributes 3% of earnings at 90 days of service.

  • The Center contributes 5% of earnings at 2 years of service.
  • Additional, 403b and 457 retirement plans available.
  • Full time employees have access to Flexible Spending Account (FSA).
