Job description

Position Overview:

The Clinician is responsible for delivering confidential counseling, education, and support to individuals, families, and the broader community while promoting healthy lifestyles and informed decision-making. Under the supervision of the Program Director or Director of Clinical Services, the Clinician provides a range of services, including but not limited to, assessments, individual and group counseling, family sessions, and other therapeutic modalities based on demonstrated expertise.

Duties and Responsibilities:
  • Conduct comprehensive client assessments using standardized or evidence-based tools and diagnose mental health disorders in accordance with DSM-5 criteria.
  • Obtain and review relevant historical records to inform current treatment planning.
  • Collaborate with clients to develop individualized, client-centered treatment plans that include measurable goals and objectives.
  • Deliver mental health counseling services aligned with the established treatment plan.
  • Monitor client progress throughout the treatment process, utilizing diagnostic criteria to assess the need for continued services, discharge, or transfer.
  • Regularly review and update treatment plans to address emerging issues and ensure ongoing relevance.
  • Develop and implement discharge or transfer plans to facilitate a seamless transition to appropriate levels of care.
  • Maintain communication and coordinate care with referral sources, including mental health providers, criminal justice agencies, educational institutions, employers, and Child Protective Services.
  • Actively participate as a contributing member of a multidisciplinary treatment team for assigned clients.
  • Provide case consultation and engage in collaborative problem-solving with colleagues and referral sources.
  • Ensure accurate and timely documentation of all counseling, education services, assessments, reassessments, referrals, and follow-up activities in the client’s clinical record, with appropriate signatures and date entries, including electronic records.
  • Positively promote the organization to prospective clients and employees by demonstrating professionalism and commitment to service excellence.
Qualifications:
  • Bachelor’s degree in a related field
  • Active clinical licensure in good standing.
  • Proficiency in computer applications and electronic health record (EHR) systems.
