Job summary

Clarvida is seeking an ACT Team Lead in San Diego, CA, to provide clinical leadership and operational oversight for a multidisciplinary ACT team serving young adults with severe mental illness.

You will supervise staff, coordinate community-based services, and ensure fidelity to the ACT model while delivering direct client care in home and community settings. This on-site role emphasizes recovery-oriented, trauma-informed practice and staff development.

Qualifications

• Master's degree in Social Work, Psychology, Counseling, MFT, or related field.
• Active California clinical license required (LCSW/LMSW/LMFT/LPCC or equivalent).
• 1–3 years of behavioral health experience with adults (SPMI).
• Supervisory/leadership experience preferred.

About the Role

The ACT Team Lead is responsible for providing clinical leadership, operational oversight, and direct supervision to a multidisciplinary Assertive Community Treatment (ACT) team serving TAY CLIENTS (16-25yrs) with severe and persistent mental illness (SPMI). This role ensures fidelity to the ACT model while supporting staff development, coordinating community-based services, and delivering direct client care through a recovery-oriented and trauma-informed approach.

Responsibilities

• Supervise and lead an ACT team consisting of Pre-Licensed Therapists, Community Integration Specialists, Co-Occurring Specialists, Peer Support Specialists, Housing Specialists, and Employment Education Specialists
• Provide clinical and administrative supervision, coaching, and performance evaluations
• Lead team meetings, case consultations, and interdisciplinary treatment planning
• Ensure adherence to ACT model standards and program requirements
• Oversee assessment, treatment planning, recovery services, and discharge coordination
• Deliver community-based outreach and direct clinical services in clients’ homes and community settings
• Respond to client crises, psychiatric symptoms, and urgent care coordination needs
• Assist with onboarding, training, and professional development of new staff
• Maintain accurate clinical documentation and ensure compliance with state, county, payer, and organizational requirements
• Collaborate closely with the Clinical Director and multidisciplinary team members to support program goals
• Perform other duties as assigned to support department, program, and organizational operations

Required Qualifications

• Master's degree in Social Work, Psychology, Counseling, Marriage and Family Therapy, or a related clinical field
• Active clinical license in California required (LCSW, LMSW, LMFT, LPCC, or equivalent)
• Full licensure preferred; associate-level licensure may be considered for exceptional candidates
• Minimum of 1 to 3 years of experience providing behavioral health services to adults with severe and persistent mental illness (SPMI)
• Prior experience in case management, community mental health, or behavioral healthcare settings
• Previous supervisory, leadership, or team coordination experience preferred
• Strong crisis intervention, clinical assessment, and risk management skills
• Excellent communication, organizational, and leadership abilities
• Knowledge of community resources, recovery-oriented practices, and multidisciplinary treatment approaches
• Proficiency with electronic health records and clinical documentation systems
