Job summary

EventusWholeHealth in Cincinnati, OH is seeking a Clinician Support Specialist to assist the clinical care team, coordinate workflows, and support telehealth and patient documentation in skilled nursing and assisted living settings.

You will collaborate with Advanced Practice Clinicians, facility staff, and families to ensure efficient, high-quality patient care, accurate scheduling, and smooth communication across care teams; travel to facilities is required.

Qualifications

  • High school diploma or equivalent is required or preferred.
  • Medical assistant/certified nursing assistant or patient care tech credentials are highly preferred.
  • Strong communication, organizational and customer service skills are essential.

Responsibilities

  • Prepare for daily clinical rounds with patient information, documentation and schedules.
  • Assist with encounter documentation, data entry and chart preparation.
  • Coordinate and maintain patient schedules including admissions, discharges and acute changes.
  • Act as a liaison between clinicians, staff, patients and families to support care delivery.
  • Assist with triage needs and documentation as directed by the clinician.
  • Organize and manage clinical documentation including labs and records.
  • Host and support telehealth visits for routine and acute care needs.
  • Obtain and process patient consents and route documentation properly.
  • Participate in facility meetings and communicate updates to the care team.
  • Support discharge planning and accurate census and records.

Position Summary

The Clinician Support Specialist (CSS) plays a critical role in supporting the clinical care team and ensuring efficient, high-quality patient care. In this position, you will work closely with Advanced Practice Clinicians (APCs) and facility staff to coordinate workflows, support clinical services, and enhance communication between patients, providers, and care teams.

Key Responsibilities
  • Prepare for daily clinical rounds by ensuring all necessary patient information, documentation, and schedules are ready in advance
  • Support clinicians in delivering care byassistingwith encounter documentation, data entry, and chart preparation
  • Coordinate andmaintainaccuratepatient schedules, including updates for admissions, discharges, and acute changes
  • Serve as a liaison between clinicians, facility staff, patients, and families to support communication and quality care delivery
  • Assistwith triage needs and documentation as directed by the clinician
  • Organize and manage clinical documentation, including lab results, diagnostic data, and patient records
  • Host and support telehealth visits for both routine and acute care needs
  • Obtain and process patient consents and ensure proper routing of documentation
  • Participate in facility meetings and communicate updates to the care team
  • Support discharge planning and ensureaccuratepatient census and records
  • Maintain professionalism and represent the organization positively within partner facilities
Qualifications
  • High school diploma or equivalent
  • Certified/Registered Medical Assistant, CNA, or PCT highly preferred
  • Minimum one year of patient care experience highly preferred, medical scribing experience preferred
  • Valid driver’s license and reliable transportation
  • Strong customer service skills with a positive, supportive, and professional approach
  • Excellent verbal, written, and electronic communication skills
  • Strong organizational skills and attention to detail; ability to navigate multiple priorities in a dynamic clinical environment
Travel Requirements
  • Regular travel to assigned facilities isrequired
  • Travel distances may exceed 60 miles from home at times
  • Reliable personal transportation isrequired
