Responsibilities

Social Worker: Inpatient

• Participates as an integral member of a multidisciplinary treatment team led by the psychiatrist.
• Facilitates the therapeutic milieu on the treatment unit.
• Performs social work interventions as directed by the treatment team, psychiatrist, and Lead Social Worker.
• Obtains and assesses social history information and reports this to the treatment teams.
• Performs discharge planning and case management from the day of admission.
• Provides group, individual, family, substance abuse, and couples therapy as deemed appropriate.
• Acts as liaison for hospitalized patients with community agencies and resources.
• Attends treatment team conferences and completes treatment plans.
• Documents in the medical record.
