Job summary

Qualifications

• Master's Degree in Social Work, Counseling, or Marriage and Family Therapy or related human service field.
• LCSW, LPC, LMFT, or LADC preferred.
• Valid Driver's License.
• Computer proficient.

Responsibilities

• Conduct comprehensive bio-psychosocial assessments and establish care plans.
• Collaborate with the interdisciplinary team to assess level of care and coordinate care/discharge plans.
• Provide direct patient care including case management and crisis intervention as needed.
• Maintain timely patient-centered documentation to meet regulatory requirements.
• Participate in administrative activities and supervision of staff.
