Job Summary:

The Providence Center Clinician/RN - Community Diversion - Providence EMS - PER DIEM supports and maintains a relationship with Providence EMS/Fire and Police Departments by performing a range of clinical and administrative functions under the direction of the Supervisor.

Provide triage and assessment services for individuals in the community experiencing a psychiatric or substance-related crisis.

Work closely with and provide education and training (individual and/or group) to community agencies, the police department, the fire department, and other first responders.

Provide outreach to appropriately referred individuals who have experienced a behavioral health event; coordinate and bridge services to behavioral health providers.

Hours: Per Diem - Flex Weekday Evenings and Weekends 9:00 AM - 5:00 PM

Duties and Responsibilities: include the following. Other duties may be assigned.

  1. Provide and coordinate with fire, police, and first responders for the provision of triage and assessment services for individuals in the community experiencing a psychiatric crisis.
  2. Identify and develop diversion opportunities and/or hospitalization for identified persons.
  3. Work closely with TPC Acute Care Programs – Emergency Services, Engagement and Diversion, Crisis Stabilization Unit, and other teams as appropriate to coordinate care and services.
  4. Facilitate collaborative relationships with the fire department, police department, and other first responders. Identify and develop training as appropriate for same.
  5. Serve as a community liaison for Community Diversion with community agencies.
  6. Collaborate with marketing to develop a communication plan to promote and market the Community Diversion program.
  7. Represent the Community Diversion program in both internal and external meetings for the organization.
  8. Organize and support the team’s daily delivery of service. Meet program goals/standards of care in accordance with Center policies and BHDDH regulations.
  9. Treat consumers with respect; promote their dignity, accept their values and lifestyles, and maintain their confidentiality in accordance with Center policy and legal requirements.
  10. Be flexible in regard to providing services within the timeframe and schedule required to best support the Fire/EMS Department and the needs of the community.
  11. Perform record keeping in accordance with Health Information Services and other Center requirements.
  12. Complete required assessment, hospitalization, and emergency certification forms, progress notes, and grant-related data tracking as applicable.
  13. Develop and implement a plan of care with ongoing consumer input.
  14. Follow required office procedures in a cooperative manner.
  15. Attend trainings, case presentations, and conferences.
  16. Provide feedback to staff on the content of educational programs attended.
  17. Attend mandatory in-service trainings and other trainings required.
  18. Participate in the education of other Center staff.
  19. Maintain cooperative relationships with Center staff, consumers, community agencies, and the public.
  20. Communicate with various local agencies and other departments or teams at The Center.

Master’s Degree in Human Services field or RN required.

Two years of relevant experience in direct care with the psychiatrically disabled population and/or in a similar social service agency.
