Job summary

Colonial Behavioral Health is seeking a Clinician III to join the Mobile Crisis Response team in Virginia. The clinician provides rapid, community-based behavioral health services to individuals in crisis, conducts comprehensive assessments, and facilitates stabilization and connections to ongoing supports.

Responsibilities include short-term crisis counseling, safety planning, and coordination with families, crisis staff, and community providers to create person-centered plans.

Qualifications

  • Master's degree in a human services field is required.
  • Resident in Counseling or Supervisee in Social Work is required.
  • DBHDS Preadmission Screening Certificate or ability to obtain within 90 days is required.

Responsibilities

  • Provide rapid, community-based response to behavioral health crises.
  • Conduct comprehensive behavioral health assessments to determine needs and risk.
  • Develop and implement person-centered safety and stabilization plans.
  • Coordinate with families, crisis staff, providers, and MCOs for continuity of care.

Job description

Colonial Behavioral Health (CBH)provides opportunities for recovery to individuals and their families affected by mental illness, substance use disorders, and developmental disabilities. CBH serves the citizens of James City County, the City of Poquoson, the City of Williamsburg, and York County.

We are currently seeking a Clinician III to join our Mobile Crisis Response program. This position provides timely, community-based behavioral health services to individuals experiencing serious mental illness and/or substance use disorders.

The Clinician III responds to individuals in crisis, conducts comprehensive assessments, facilitates stabilization and appropriate treatment, and connects individuals with the services and supports necessary to promote safety, recovery, and ongoing stability.

Job Responsibilities
  • Provide rapid, community-based response to individuals experiencing behavioral health crises in settings where they are most comfortable and where services can be most effectively delivered.
  • Respond to a variety of community locations, includingschools, private residences, places of employment or education, and other community settings, to facilitate engagement, stabilization, and crisis resolution whenever possible.
  • Conduct comprehensive behavioral health assessments to determine:
    • Appropriateness for services and level of care.
    • Risk of harm to self or others.
    • Risk related to inability to care for oneself due to mental illness.
    • Functional status and current level of stability.
    • Medical, psychiatric, and substance use co-occurring conditions.
    • Recovery environment and available supports.
    • Treatment and recovery history.
    • Ability and willingness to engage in recommended services.
  • Provide clinically appropriate interventions for individuals experiencing acute behavioral health crises, including short-term crisis counseling, safety planning, hospitalization, triage, referral, and other appropriate dispositions.
  • Collaborate with individuals, families, crisis staff, community providers, Managed Care Organizations, and other collateral contacts to develop and implement effective, person-centered plans for safety and stabilization.
  • Develop crisis Individual Service Plans (ISPs) and other treatment and safety plans designed to support immediate stabilization and ongoing recovery.
  • Transition individuals from Mobile Crisis Response to the most appropriate level of care, ensuring timely care coordination and communication with receiving providers and other involved supports.
  • Maintain effective communication and coordination withRegion 5 Northern Hub.
  • Follow applicable discharge protocols and continuity-of-care procedures between Community Services Boards, state psychiatric facilities, private psychiatric facilities, and community-based providers.
  • Maintain accurate and timely clinical documentation in accordance with agency, state, and regulatory requirements.
  • Perform other duties and responsibilities as assigned.

This position requires flexibility to work days, evenings, nights, weekends, and holidays based on program and community needs.

Skills and Qualifications
  • Knowledge and clinical skills in theassessment, evaluation, and diagnosisof individuals experiencing mental illness, intellectual/developmental disabilities, and substance use disorders.
  • Ability to assess and determine risk, includingdanger to self or others, inability to care for oneself due to mental illness, and imminent risk.
  • Ability to provide an effective and clinically appropriate response to individuals experiencingacute behavioral health crises.
  • Knowledge of crisis intervention practices, including assessment, short-term crisis counseling, hospitalization, triage, referral, and other appropriate dispositions.
  • Knowledge and understanding ofdischarge protocols and continuity-of-care proceduresinvolving Community Services Boards, state psychiatric facilities, private psychiatric facilities, and community providers.
  • Understanding of and ability to follow Emergency Services procedures, applicable CBH policies and procedures, and relevant state requirements.
  • At least one year of clinical experience with individuals with serious mental illness and/or substance use disorder required.
  • Valid driver’s license/record is required.
  • Master’s Degree in a human services field required.
  • Resident in Counseling or Supervisee in Social Work, required.
  • DBHDS Preadmission Screening Certificate or ability to obtain within 90 days required.
