Program Description

Graham Windham’s Community Mental Health & Wellness Clinic is an Article 31 OMH program that offers a variety of evidence‑based treatment for children, youth, young adults, and their families from the age of 5‑25 years of age, based on our comprehensive psychiatric and biopsychosocial evaluations. We offer individual therapy, family therapy, group therapy, psychological and psychiatric evaluations, psychopharmacology management, and support services. Our Harlem clinic offers walk‑in hours for immediate mental health assessments. Each treatment plan is created in collaboration with the client based on the needs of the individual client. We also offer bilingual services in Spanish to better support the needs of our communities.

Job Responsibilities

• Provide individual, group, family, and collateral treatment and crisis services/interventions that are focused and goal oriented; offer information and counseling on the best course of action during sessions.
• Apply the appropriate evidence‑based psychotherapeutic interventions to assess, monitor, and ameliorate symptoms; minimize the negative effects of trauma symptoms, behaviors, and environmental stressors.
• Provide, implement, and coordinate comprehensive initial and ongoing assessments and treatment plans for new intake clients and ongoing clients on the caseload.
• Collaborate with clients, collaterals, and their multidisciplinary clinical treatment team in developing, reviewing, and modifying treatment plans; monitor and evaluate clients’ progress and modify plans as needed.
• Consult with other behavioral health, primary care, and specialty care providers as appropriate. Collaborate with outside organizations such as schools, hospitals, higher‑level‑of‑care programs, and other service programs on behalf of client’s care needs.
• Provide engagement and rapport to foster individuals’ commitment to the therapeutic relationship and regular therapy attendance. Explore motivation, resistance, and attendance as needed.
• Maintain electronic records and documentation according to the standards and time frames established by OMH, Medicaid regulations, and Graham policies and procedures.
• Maintain expected levels of productivity and provide quality care as defined by regulatory agencies and Graham.
• Provide crisis intervention services to de‑escalate acute distress situations; manage reportable incidents and complete necessary documentation.
• Coordinate the discharge planning process, identify resources and support needed for transition to another program, and make referrals, linkages for treatment, rehabilitation, and support services.
• Be actively self‑reflective and self‑aware of how clinical and administrative practice affects clients, families, and the communities we serve; be open and willing to receive constructive feedback.
• Approach work with an anti‑racism lens and continuously seek learning and professional growth.
• Participate in clinical meetings, staff meetings, trainings, supervision, case conferences, etc.
• Maintain professional behaviors and ethical standards as established by the licensing board and Graham. Uphold the privacy of patient, client, or protected member health information.

Qualifications

• Master’s degree with a professional license: LMSW, LCSW, LMHC, LMFT, LCAT
• Limited permit holders eligible.
• MSW permit holders must obtain a license within 1 year.
• MHC/MFT/CAT permit holders must obtain a license within 2 years.
• 1 year of post‑masters clinical experience with children, adolescents, and families.
• Ability to work evenings and/or Saturdays.
• Bilingual Spanish preferred; an additional salary differential will be given to employees who demonstrate adequate proficiency through a language exam.
• High degree of computer literacy.
• Experience in an Article 31 Mental Health Clinic preferred.
• Demonstrated experience with diverse populations preferred.

Salary & Compensation

Hourly Rate for Non‑Billable Time: $16.50–$26.00/hour

Hourly Rate for Billable/Client Service Time: $54.00–$64.50/hour
