Job description

The Clinical Care Coordinator is responsible for developing and providing high-quality therapeutic interventions and services in support of an individual’s wellness and recovery. Services include strength-based and needs-based therapeutic case management, problem-solving, therapeutic rehabilitation including skills-building, benefits support, resource development, housing navigation, crisis intervention, and family support. Schedules may include weekends as needed by the program.

Schedule: Monday through Friday. Day shift.

DUTIES AND RESPONSIBILITIES:

  1. Delivery of individual, family, and group counseling, individual rehabilitation, and case management/brokerage for individuals within the Care Coordinator's caseload as assigned.
  2. Utilize Evidence-Based Practices for the designated population/program to ensure high-quality results and intended impact.
  3. Assess an individual’s risk status, including risk-based symptoms, substance use, environment, physical conditions, etc., and intervene appropriately.
  4. Create a service plan that aims at an individual getting better/healthy and accomplishing service goals that are person-centered and belong to the client.
  5. Engage in outreach, report disengagement immediately, and work with the outreach team to determine when to disenroll individuals based on program protocols.
  6. Conduct assessments, screenings, referrals, service planning, community planning, and benefits coordination to support wellness and recovery principles.
  7. Document and maintain all data collection, reporting, and charting records in accordance with BACS and other relevant county regulatory policies and agencies.
  8. Participate in supervision, staff meetings, consultations, travel, crisis/emergency coverage, weekend coverage, and agency-wide meetings and training as required by the manager.

QUALIFICATIONS:

  • ASW, AMFT, or APCC with current BBS registration; and at least 1 year of social service experience.
  • LCSW, LMFT, or LPCC with current BBS registration; and at least 1 year of social service experience.
  • RN with current CA license and at least 1 year of social service/psychiatric experience.
  • Bilingual in a threshold language preferred.

  • Highly competitive compensation: $40/hourly for non-licensed but BBS-registered AMFT, ACSW/APCC; $104,000 annually (exempt) for licensed BBS-registered LCSW/LMFT/LPCC.
