The role emphasizes delivering a positive patient experience and coordinating with POD teams to schedule appointments as needed. The position requires RN or LPN licensure (PA/NJ or compact state) and at least 2 years of recent clinical experience in relevant settings.

Qualifications

• CTS RN - Graduate of an accredited nursing program with active RN licensure in PA/NJ or other compact state.
• CTS LPN - Graduate of an accredited nursing program with active LPN licensure in PA/NJ or other compact state.
• Minimum 2 years of recent clinical experience in physician offices, home health, critical care or emergency room.

Responsibilities

• Answers telephones, prioritizes clinical triage calls, follows clinical protocols and coordinates services.
• Verifies patient demographics and updates information into the EHR.
• Acts as escalation point for clinical patient issues and provides advice based on protocols.
• Manages escalated electronic patient messages with clinical judgment.
• Documents symptoms, nursing assessment, advice given and patient response.
• Collaborates with Access Center teams and practice clinical staff to assist with concerns, refills, or scheduling.
