Clinical Transition Nurse – Hospice Care (Murrieta, CA)

About The Role

A mission-driven healthcare provider is seeking a dedicated Clinical Transition Nurse (CTN) to support patients and families transitioning into hospice care. This role focuses on care continuity, symptom management, and compassionate guidance during end-of-life transitions.

Key Responsibilities

  • Conduct comprehensive assessments of physical, emotional, and spiritual needs.
  • Collaborate with Clinical Teams: Collaborate with clinical teams by actively sourcing hospice referrals from healthcare providers in the medical facilities.
  • Create and manage individualized care plans focused on comfort and quality of life.
  • Collaborate with physicians, chaplains, social workers, and other team members.
  • Educate families about hospice services, symptom control, and care processes.
  • Oversee transitions from pre-admission through inpatient hospice admission when applicable.
  • Monitor patient progress and adjust care as needed.
  • Support referral development and review insurance and consent documentation.
  • Promote the organization’s values and compassionate service culture.

Qualifications

  • Graduate of an accredited nursing program; BSN preferred.
  • Active RN license in California and current CPR certification.
  • 1+ year hospice experience and 1–2 years in acute/home care.
  • Must have valid driver’s license, insured vehicle, and ability to travel locally.
  • Strong communication, problem-solving, and care coordination skills.
  • Holistic care training is a plus.
