Who We Are Looking For:
Every day, Cambia’s dedicated team of Clinical Transformation Advisor s are living our mission to make health care easier and lives better. As a member of the Care Management team, our Clinical Transformation Advisors drive healthcare transformation in quality, cost and consumer experience by working with key strategic provider partners to enable their success in Cambia’s value-based arrangements and transformation initiatives. Meets regularly with providers to facilitate practice transformation efforts through hands-on expertise in quality management and clinical process improvement. Works in close partnership with the Provider Partnership Manager and other team members to develop and implement transformation initiatives with providers and to support accountable care products and membership growth – all in service of creating a person-focused health care experience.

What You Bring to Cambia:
Qualifications:
Bachelor’s Degree in Nursing or related field (Masters preferred)
4+ years’ experience or equivalent combination of education and experience in one of the following areas: population health, care management, program management, relationship management or clinical leadership.
Strong preference for experience within a medical practice or facility
Current LPN or RN license, in a state or territory of the United States
Skills and Attributes:
Relationship Building & Communication: Customer-focused mindset with proven ability to build positive relationships, gain trust and respect, and influence stakeholders at all organizational levels. Strong communication and facilitation skills for discussions, presentations, and consensus-building among diverse groups, including provider management on clinical value-based arrangements.
Problem-Solving & Project Leadership: Demonstrated ability to identify problems, develop solutions, and implement action plans to resolve complex issues. Proven experience managing and leading complex projects with strong analytical and problem-solving capabilities.
Data Analytics & Technical Proficiency: Experience interpreting and utilizing clinical data analytics and outcomes measures to drive change, with skills to analyze and present complex data. Proficient in word processing and spreadsheet software for creating and maintaining databases, reports, and documents.
Healthcare Industry Expertise: Knowledge of industry trends, products, and solutions related to healthcare transformation and value-based arrangements, including understanding of provider billing practices and healthcare reimbursement methodologies.
Work Style & Flexibility: Ability to work independently and collaboratively within teams, with willingness to travel to provider locations as needed.
What You Will Do at Cambia:
Partnership Development & Stakeholder Engagement: Develops collaborative, productive partnerships with key providers and internal stakeholders; participates in executive-level Joint Operating Committees and internal/external work groups; attends external meetings to represent Cambia's position on local and national provider programs
Clinical Performance & Value-Based Care Expertise: Maintains detailed knowledge of clinical performance levers (care gaps, Medicare STARS, HCC coding) and value-based arrangements including contract terms and performance targets; provides input on quality metrics for new arrangements and future PPI strategies; serves as subject matter expert on competitive intelligence regarding clinical interventions in accountable care
Provider Education & Clinical Guidance: Researches, develops, and conducts provider education, training, and presentations to communicate vision for change; applies clinical practice understanding to provide guidance on infrastructure, systems, and processes; teaches providers to use quality and utilization tools for performance management
Clinical Improvement & Care Coordination: Identifies clinical improvement opportunities for value-based arrangements; leads initiatives to help providers design and implement processes for higher quality outcomes, better care management, and improved member experience; leverages Cambia's clinical resources to enhance provider care coordination capabilities
Leadership & Strategic Integration: Provides leadership to provider-aligned nurses and care advocates supporting value-based programs; supports Provider Partnership Engagement Manager and Provider Partnership Manager by integrating clinical interventions with overall value-based strategy; partners with Clinical Services and Medical Directors to implement key deliverables; promotes peer-to-peer physician collaboration with Executive Medical Directors

The expected hiring range for a Clinical Transformation Advisor is $75,700 ($78k WA) - $102,400 depending on skills, experience, education, and training; relevant licensure / certifications; and performance history. The bonus target for this position is 10%. The current full salary range for this role is $71,000 ($78k WA) - $116,000.
