Job Overview

Pomona TAY program - Transitional age youth is ages 16-25, referral and criteria only based. The program is an 18-24 months intense stabilizing program aimed at medication management, stabilization, reducing hospitalizations, and preparing clients for step-down care. Clients must be referred via DMH and meet specific criteria (psychosis, homelessness, hospitalization history, co-occurring disorders). FSP is the highest care level before psychiatric hospitalization, involving high-intensity, field-based services with weekly meetings with therapists and case managers. The goal is to foster independence and stability before graduation or stepping down.

Key Responsibilities
  • Coordinate care for TAY clients receiving mental health services.
  • Collaborate with Probation, SUD staff, and Case Managers on treatment plans.
  • Meet weekly productivity expectations.
  • Complete documentation within 24 hours.
Qualifications and Skills
  • Degree in MFT, MSW, or Ph.D./Psy/D. from an accredited graduate program.
  • Minimum two years of clinical experience providing direct mental health services to children.
  • Experience with evidence-based practices and trauma-informed care preferred.
  • Familiarity with Los Angeles County DMH documentation preferred.
  • Flexible work schedule, including evenings/weekends, preferred.
  • Ability to pass background and criminal checks.
  • Reliable transportation, valid California Driver's License, and proof of insurance required.
  • Bilingual in English/Spanish preferred.
  • Must be registered with the BBS, licensed, or license-eligible.
