Summary

The clinical therapist will provide mental health therapy services to youth/children and adults with Intellectual/Developmental Disabilities (IDD) within an integrated health clinic. This position requires supporting the case manager in the formulation of an individualized treatment plan, which will be used in conjunction with the entire primary care team to develop a holistic care strategy for each client.

Requirements And Duties

1. Organize workload by prioritizing work assignments to meet consumer and program needs efficiently.
2. Provide psychotherapeutic support/counseling to clients according to DSM 5 guidelines.
3. Discuss continuing care needs with the multidisciplinary care team.
4. Ensure proper handling and compliance of confidential medical records (EMR).
5. Maintain record requests for medical records from other sources.
6. Plan follow-up for clients with post-acute transitional concerns.
7. Document assessment findings in SW outpatient assessment or SW progress notes within EMR.
8. Communicate findings and transition plans with the care team (nurse, physician, case management, etc.).
9. Complete administrative forms for billing, data collection, and management information systems in accordance with agency policy.
10. Document all consumer/guardian and collateral contacts in compliance with agency policy.

Requirements

Certificates, Licenses, Education:

• MSW - Master of Social Work
• Required Licensure - LMSW, LCWS, LMFT

Hybrid Work Policy

This position offers a hybrid work schedule, contingent on meeting productivity and telehealth standards. During the first 30 days, staff are required to work in the office, with possible extension based on performance.

Employees may work remotely part of the time, but must be flexible to client needs, including in-person appointments.
