Offer Based on Years of Experience

Part-time PRN less than 25 hours per week (between M-F 8-5pm CST)

Current LVN or RN for MS and/or TN Required

HCHB & Epic experience strongly preferred

Salary Range: $25/hr-$33/hr

What You Need to Know

The Territory Patient Admission Coordinator is responsible for the timely acceptance or decline of referrals based on the predetermined criteria set by the agency. This position's assigned territory may encompass one or more states in AccentCare's operation and may change with the needs of the company. The coordinator establishes and maintains communication with clients, family members, and referral sources. This role partners with agency staff, referral sources, and sales to achieve optimal patient satisfaction and outcomes. It facilitates obtaining all necessary information and documentation to complete a referral through follow-up with the agency, referral source, and client.

If you meet these qualifications, we want to meet you!

• Licensed RN, LVN, or PT in practicing state
• 3-7 years of experience in facility/physician relationships, with a deep understanding of facility discharge processes preferred
• Minimum of one year experience in home care, hospice, or personal care services preferred
• Prior use of Electronic Medical Records (EMR) preferred
• Experience dealing with various healthcare payors (Medicare, Medicaid, commercial payers) preferred
