Job Overview :
As the Team Lead, you will be responsible for overseeing a group of Diabetes Educators and Dietitians who deliver telehealth services to our patients. You will manage the team’s performance, provide mentorship, ensure adherence to clinical protocols, and work closely with management on strategic initiatives. In addition to your leadership role, you will continue to serve as a provider, delivering care to patients and directly contributing to their education and therapeutic needs.
Key Responsibilities :
- Supervise and support a team of up to 20 remote Diabetes Educators and Dietitians, ensuring they provide high-quality, patient-centered care.
- Conduct monthly one-on-one check-ins and annual performance reviews to provide guidance, feedback, and professional development.
- Monitor and evaluate team performance, ensuring adherence to company guidelines, clinical best practices, and performance metrics.
- Foster a collaborative, supportive team culture, encouraging communication, knowledge-sharing, and mutual respect.
- Resolve provider challenges, including clinical, operational, and interpersonal issues, and escalate to management when necessary.
- Collaborate with HR and management to facilitate onboarding and training for new hires.
- Ensure the team stays up-to-date on diabetes care practices, Medical Nutrition Therapy protocols, QA initiatives and findings, and telehealth technologies.
Patient Care (20%) :
- Provide direct care to patients as a Diabetes Educator and/or Dietitian, including conducting virtual consultations, education sessions, and developing personalized care plans.
- Lead diabetes education sessions, addressing topics such as blood glucose management, meal planning, exercise, and medication adherence.
- Collaborate with patients to set achievable goals and monitor their progress, adjusting care plans as needed.
- Document all patient interactions thoroughly and accurately in the electronic health record (EHR) system.
- Provide follow-up support to patients to ensure their continued success in managing diabetes through regular check-ins and assessments.
- Work with management to identify and implement strategic initiatives to improve patient outcomes and enhance team performance.
- Contribute to the development of training materials, workflows, and resources for the team of providers.
- Participate in weekly Leads meeting with Director(s) to discuss operational improvements, patient satisfaction, and team goals.
- Help design and implement new projects to increase efficiency, improve care delivery, and enhance the patient experience.
- Analyze and report on key performance indicators (KPIs) and team metrics to management, suggesting improvements and identifying trends.
Qualifications :
- Registered Dietitian (RD) and/or Certified Diabetes Care and Education Specialist (CDCES) required.
- Minimum of 3-5 years of experience in diabetes education, with at least 2 years in a leadership or supervisory role.
- Experience providing telehealth services is highly preferred.
- Strong leadership skills, with the ability to inspire and manage a remote team.
- Excellent communication skills, with the ability to provide clear instructions and feedback to both team members and patients.
- Proficient in using Electronic Health Records (EHR) and telehealth platforms.
- Ability to manage multiple priorities and projects in a fast-paced environment.
- Knowledge of diabetes management, current research, and clinical guidelines.
- Strong organizational skills and attention to detail.
Preferred :
- Master’s degree in nutrition, public health, or related field.
- Experience working in a telehealth environment or remote work setting.
- Bilingual in English and Spanish or other languages is a plus.
