Job summary

Atlantic Health System is seeking a System Pharmacist - Clinical for Internal Medicine/Transition of Care. This patient-care focused role provides pharmacotherapy recommendations and collaborates with care teams across sites, with on-site work at the Morristown corporate office and opportunities for remote patient care.

Qualified candidates hold a PharmD, have PGY1 residency or 2 years of clinical pharmacy experience, and possess active New Jersey licensure.

Qualifications

• Graduated from accredited School of Pharmacy with PharmD.
• PGY1 residency or 2 years of clinical pharmacy experience in the assigned area.
• Current active registration and licensure with the State Board of Pharmacy in New Jersey.

Responsibilities

• Review and verify medication orders for appropriateness, safety, and accuracy.
• Monitor therapeutic outcomes and provide clinical recommendations to the care team.
• Collaborate with healthcare professionals across sites to optimize patient care.
• Participate in quality improvement and patient safety initiatives.
• Ensure compliance with hospital, departmental, and regulatory policies.
• Document interventions and recommendations in the EHR.
• Stay current with developments in pharmacotherapy and evidence-based medicine.
