Job summary

Hospice & Community Care is seeking an RN-Clinical Support in an in-office role, offering a 40-hour work week with shift options (8am-5pm or 9am-6pm) and no holidays or weekends. This role triages calls, coordinates follow-up, and provides education on end-of-life care, coordinating transfers and updates to patient records.

Responsibilities include obtaining refills, coordinating symptom management orders with contracted pharmacies, and mentoring hospice staff, with orientation of 5–6 weeks

Qualifications

• Registered Nurse license in Pennsylvania.
• Associates in nursing required.
• Minimum of two years hospice, medical/surgical, home health, or relevant experience.

• Triages calls assessing pain and symptoms, physical needs and disease progression.
• Provides direction to the caller or coordinates necessary follow-up, prioritizing for those with uncontrolled pain/symptoms and imminent death.
• Provides anticipatory guidance and education for disease progression and end of life care. Utilizes patient teaching tools and handbooks as reference.
• Assists with telephonic assessment and coordination of inpatient center transfers.
• Obtains refills, renewals, new and revised orders for medication and treatments.
• Supports the initiation of symptom management orders through timely notification to contracted pharmacy.
• Updates the clinical record with new orders, medications and treatments, and communicates to team. Anticipates medication, DME and supply needs and bundles orders.
• Serves as a resource and mentor to hospice clinical staff

RN- Clinical Support
