Job summary

Amazing Care Home Health Services in Westminster is seeking an RN Clinical Supervisor to lead and elevate our Skilled Nursing team, ensuring high-quality, patient-centered care.

You will mentor field clinicians, oversee admissions, recertifications, and care planning, and collaborate with case managers to drive outcomes in a dynamic home health environment.

Qualifications

• Active RN license
• Proven experience in home health and OASIS assessments (required)

Responsibilities

• Oversee patient care across admissions, recertifications, routine visits, transfers, and discharge planning
• Provide clinical leadership, mentorship, training, and support to field clinicians

• Balance administrative oversight with hands-on clinical support, including occasional patient visits
• Help drive quality, compliance, and best practices (including OASIS accuracy)
• Adapt to a dynamic environment with a proactive, solutions-focused mindset

Compensation

• Salary: Approximately $97,000+ (based on experience)
• Sign-On Bonus: $5,000

What You'll Do

• Oversee patient care across the full continuum: admissions, recertifications, routine visits, transfers, and discharge planning

Requirements

• Active RN license
• Proven experience in home health and OASIS assessments (required)
