21st Century Home Health Services (21HHS) is seeking a Clinical Supervisor: Therapy to lead patient care coordination for in-home visits in the San Jose area. You will supervise field clinicians, ensure timely support, and maintain high-quality clinical services delivered at patients' homes.

This leadership role blends office and field work, with 2–3 days in the San Jose office and 3 days in the field, overseeing 15+ patient visits weekly and guiding clinical staff to optimal outcomes.

What You’ll Do:

The Clinical Supervisor - Therapy is responsible for ensuring that patient care is coordinated and managed appropriately. They serve as the first point of contact for field clinicians to ensure support is easily accessible in a timely fashion. The Clinical Supervisor-Therapy is responsible for ensuring that care and services are delivered appropriately as well as the supervision of clinical personnel. This is a leadership position that is responsible for the oversight of clinicians providing patient care to patients in their place of residence.

This is a hybrid of field work and on site based at 21HHS office:

2- 3 Days in the San Jose office

3 days in the field: Average 15+ patients visits each week (supervisory visits of field clinicians)

• You will be assigned a primary location; however, you may occasionally be asked to provide coverage outside of that area. This may include supporting vacancies, leaves of absence, higher patient census, or other client care needs.
