Overview

The Clinical Supervisor is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Supervisor oversees the delivery of care and supervises clinical personnel.

Responsibilities
  1. Receive and review case referrals, including home visits, to determine hospice/home care needs. Assign appropriate personnel and consult with the Medical Director regarding patient eligibility.

  2. Evaluate each case through home visits, record reviews, and clinician services. Guide clinicians to improve performance and ensure quality care. Be available during operating hours for clinician support.

  3. Review patient's medical diagnosis, prognosis, medications, procedures, and clinical course.

  4. Assist clinicians in establishing therapeutic goals, setting priorities, and developing care plans.

  5. Participate in case conferences to coordinate care and discuss interdisciplinary involvement.

  6. Conduct quarterly record reviews, communicate findings, and recommend improvements to the Clinical Director and staff.

  7. Assist in screening, interviewing, and onboarding new employees. Contribute to orientation programs.

  8. Support the Clinical Director and team in planning, implementing, and evaluating educational programs and organizational policies.

  9. Ensure compliance with professional standards and principles.

  10. Assess community health needs and participate in developing and promoting programs to meet these needs. Engage in public relations and community activities to enhance the organization's role and promote customer service.

  11. Participate actively in quality assessment and performance improvement initiatives.

Qualifications
  • Current licensure as a registered nurse in the state.

  • BSN degree from an NLN-accredited school preferred.

  • Experience in hospice/home care and at least two (2) years in management or supervision.

  • Valid driver’s license, insured vehicle, and good driving record.

  • Current CPR certification.

  • Proven ability to work within an interdisciplinary team.

  • Adherence to professional standards and practices.

  • Excellent observation, judgment, communication skills, and understanding of hospice philosophy and issues related to death and dying.

  • Knowledge of pain and symptom management principles.

  • Strong verbal and written communication skills.
