Job summary

Enhabit Home Health & Hospice is seeking an experienced nurse leader to oversee home health services, ensuring care coordination and regulatory compliance. The role focuses on collaboration with physicians, patients, and staff to deliver high-quality care in the home setting.

The position requires an RN license, CPR, and at least two years of nursing experience with one year in home health. Strong communication and technology skills are essential for success in this role.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical nursing license for state requirements (or compact license).
• Minimum two years of nursing experience within the last 36 months.
• Minimum one year of home health experience.
• Knowledge of federal/state/local regulations governing home care operations.
• CPR certification.
• Valid state driver license and automobile liability insurance.
• Demonstrated technology skills, including mobile devices and MS Office.

Responsibilities

• Coordinate overall home health services delivered to patients per standards and procedures.
• Review and approve patient information at start of care, recertification, or evaluation visits.
• Facilitate collaboration among physicians, referral sources, patients, caregivers, and staff to maintain care continuity.
• Schedule patients to field clinicians and communicate with all stakeholders to ensure proper care coordination.
