Job summary

Enhabit Home Health & Hospice in Georgia is seeking an experienced nurse leader to oversee the coordination of in-home health services, ensuring care meets accepted standards and patient needs are met. The role requires strong collaboration with physicians, referral sources, patients, and caregivers, with responsibility for scheduling and maintaining continuity of care across the team.

In-office attendance is essential to lead staff, supervise operations, and drive quality outcomes;

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical nursing license, per state requirements.
• CPR certification.
• Minimum two years nursing experience in the previous 36 months.
• Minimum one year of home health experience.
• Knowledge of federal, state and local laws governing home care.
• Valid state driver's license and automobile liability insurance.
• Proficient technology skills, including Microsoft Word and Excel.
• Management experience preferred.
• Bilingual English/Spanish preferred.
• Dependable transportation and ability to drive in all weather.
• Oral and written communication skills.

Responsibilities

• Coordinate home health services for patients per standards.
• Review and approve start of care, recertification, resumption or evaluation documents.
• Facilitate relationships with physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and ensure continuity of care.

Responsibilities

Ensure the overall coordination of home health services provided to the patient is delivered according to acceptable standards of practice and company procedures. Review and approve patient information submitted by the licensed professional during a start of care, recertification, resumption of care, or evaluation visit. Facilitate the relationship between physicians, referral sources, patients, caregivers, and employees. Ensure scheduling of patients to field clinicians and communication with field staff, patients, physicians, referral sources, caregivers, and other service providers in order to maintain proper care coordination and continuity of care.

Qualifications

• Required graduate of an approved school of professional nursing.
• Required to have and maintain a valid clinical license, per state requirements, or in accordance with the board of nursing rules for a compact nurse license.
  • Experience as a licensed professional may be deferred with a review and approval from the vice president operations or regional president
• Required to have previous experience practicing under the professional license for this position.
• Required to have minimum of two years of nursing experience within the immediate previous 36 months.
• Required to have minimum of one year of experience with home health.
• Required to have demonstrated knowledge and understanding of federal, state and local laws and regulatory guidelines that govern the operation of a home care office.
• Required to have and maintain CPR certification, as required by position.
• Required to have and maintain a valid state driver license in the current state of residence
• Required to have and maintain automobile liability insurance, as required by law
• Required to have basic demonstrated technology skills, including operation of a mobile device.
• Preferred previous experience in a management position.
• Preferred previous experience using Homecare Homebase (HCHB).
• Preferred bilingual (English/Spanish) with the ability to facilitate clear communication with patients and caregivers, improve service delivery, and support positive patient outcomes.
• Must maintain dependable transportation in good working condition and be able to safely drive in all types of weather conditions.
• Oral communication, written communication, fluency in English, active listening.
• Demonstrated intermediate technology skills. Especially Microsoft office, including word and excel.
• Other than for designated full-time remote positions, in‑office attendance is essential in order to achieve Enhabit objectives, including but not limited to, effective supervision, teamwork, task completion, and other position objectives.
