Job Description

Schedule: Full-Time, Day Shift, 40 hours/week. Monday through Friday.

Summary
  • Under the direction of the Senior Practice Manager, the Clinical Supervisor is responsible for the ongoing development, implementation, management and evaluation of support service programs that primarily serve individuals living with HIV. This position provides both administrative and clinical leadership while maintaining a small direct service caseload, ensuring that program operations remain aligned with the evolving needs of patients, staff and community partners. This position will be instrumental in building, expanding and strengthening the Support Services Team to ensure program growth, quality service delivery and compliance with all regulatory and grant requirements. The Clinical Supervisor provides direct supervision to Social Workers, Medical Case Managers, Case Managers, Outreach Staff and other assigned personnel and is responsible for supporting both the clinical and operational needs of the department. In addition to supervisory and programmatic responsibilities, the Clinical Supervisor will maintain a limited direct service caseload. The incumbent will provide clinical support, care coordination, resource navigation and case management services to ensure ongoing connection to care and to maintain direct knowledge of patient and community needs. This responsibility supports program quality, staff mentorship and the continued development of patient-centered services. The Clinical Supervisor is responsible for overseeing daily operations, supporting staff development, monitoring quality and productivity standards and fostering innovation through the development of standard work, performance metrics and best practices.
Responsibilities
  • Provides clinical and administrative oversight to the Support Services Team, including Medical Case Managers, Case Managers, Social Workers, Outreach Staff and other assigned personnel. Responsible for coordinating daily departmental operations and ensuring effective service delivery.
  • Maintains a limited caseload, providing direct case management, care coordination, resource linkage, patient advocacy and treatment planning as appropriate. Utilizes direct service experience to inform program development, staff supervision and quality improvement initiatives.
  • In collaboration with the Senior Practice Manager, the Clinical Supervisor assists in the development, expansion and implementation of support service programs designed to improve health outcomes and access to care for individuals living with HIV.
  • Provides ongoing coaching, supervision and professional development to promote staff growth and ensure adherence to program standards and organizational expectations.
  • Conducts individual and group supervision meetings with Support Services staff to establish standard work, monitor performance metrics, address barriers and foster innovation and continuous improvement.
  • Provides clinical guidance to staff and assists with complex patient situations, resource coordination, patient advocacy and treatment planning activities.
  • In collaboration with the Senior Practice Manager, the Clinical Supervisor oversees grant-funded programs and services, ensuring compliance with grant requirements, performance measures, reporting obligations and contractual expectations. Maintains master files and documentation related to grants and funded programs.
  • Develops and prepares required reports and program updates.
  • Maintains procedures, workflows and quality monitoring plans to ensure compliance with organizational standards, grant requirements and regulatory expectations.
  • Coordinates and facilitates project meetings, prepares status reports and supports problem solving related to assigned programs and initiatives.
  • Leads the development and implementation of new and revised policies, procedures and workflows related to Support Services and grant-funded programs.
  • Develops and maintains collaborative relationships with community agencies, healthcare providers, funding organizations and key stakeholders. Facilitates communication between community partners, patients, families and clinical personnel.
  • Assists leadership with departmental planning, resource allocation, budget development and strategic initiatives related to Support Services expansion and program growth.
  • Promotes staff competency, continuing education and compliance with professional licensing and certification requirements.
  • Oversees documentation standards and ensures care activities are documented according to department policies, grant requirements and regulatory standards.
  • Acts as a patient advocate and supports initiatives that improve access to care, retention in care and health outcomes for patients served by the program.
  • Complies with agency guidelines, policies and procedures.
  • Complies with applicable professional licensing requirements and ethical standards.
  • Attend and participate in staff, committee, department and administrative meetings.
  • Attend educational conferences, trainings and professional development activities.
  • Pursue knowledge, best practices and innovative models that advance Medical Case Management and Support Services programs.
  • Participate in organizational initiatives, strategic planning activities and special projects.
Credentials
  • LCSW or LPC or MFT or NCC or PHD or PN or RN or PS or RN or
  • LSW, LCSW, LPC or equivalent professional credential in good standing with the appropriate Pennsylvania licensing board is required.
  • Cert Alcohol and Drug Cnslr
  • Certification in Case Management, Drug and Alcohol Counseling or related fields are preferred.
Education or Equivalent Experience
  • Master of Arts or Science (Required)
