Institute on Aging in San Mateo, CA, is seeking a Clinical Supervisor for the Community Living Fund (CLF) Intensive Care Management staff on a temporary, per diem basis. The role supports operations up to 19 hours per week and oversees master’s level social workers, occupational therapists, and others.

Responsibilities include supervision, staff training, regulatory compliance, care planning, and coordinating with partner agencies to ensure high-quality, timely care and documentation under CLF

The primary responsibility of the Clinical Supervisor is to provide clinical supervision of Community Living Fund (CLF) Intensive Care Management (ICM) staff including Master’s level Social Workers, Occupational Therapists, Money Managers, and others.

This is a temporary, per diem role designed to provide operational support on an as-needed basis. Hours are variable, up to a maximum of 19 hours per week, based on department needs.

Key duties and responsibilities include, but are not necessarily limited to the following:

• Responsible for the clinical supervision, training, and evaluation of the Care Management staff participating in the Community Living Fund in collaboration with Partner agencies.

• Participates in the hiring and termination of CLF Care Management staff.

• Facilitates the day-to-day operations, ensuring appropriate systems are in place.

• Monitors program and staff compliance with all regulatory requirements, such as San Francisco Department of Aging and Adult Services/San Francisco Human Services Agency and Partner organizations.

• Ensures high quality, timely care, and service delivery.

• Implements and monitors multiple systems, including records, clinical documentation and personnel. Ensures that documentation is compliant with regulations, policy and procedure manual, and is complete and accurate.

• Organizes and conducts regular care planning meetings and supervision.

• Facilitates the orientation and training of all new staff, including joint home visits, supervisory meetings, and documentation review.

• Responsible for assessing the educational needs of staff, developing and coordinating an education and training plan that assures a high level of skill and competency in the understanding of case management, community resources, and family systems.

• Establishes and implements Quality Assurance mechanisms to measure and maintain high standards of care and programming, including regular chart reviews and peer review.

• Develops and maintains collaborative working relationships and cooperative arrangements with community individuals, group ororganization; takes advantage of opportunities to educate the public and the community regarding CLF services.

• Responsible for remaining current with changes in San Francisco community resources, Medi-Cal and Medicare regulations and procedures that will impact the practice and direction of the program; acts as a resource to CLF and IOA staff.

• Responsible for ensuring CLF purchase of service funds are used as a last resort and all community resources and referrals are made prior to the authorization of CLF funds; verifies CLF funds are used to prevent institutionalization when there are gaps in client care.

• Provides back-up coverage for clinical staff.

• Works with local schools to secure and to supervise social work interns.

• In collaboration with the Director of the Community Living Fund, participates in establishing annual goals and objectives relevant to the needs of the program and in planning, developing and evaluating policies and procedures; actively involves staff in the process.

• Actively participates in and encourages actions that promote good public relations with clients, their families, and the community.

• Responsible for expanding personal knowledge, skill and attitudes specific to care management, family systems, geriatrics and gerontology, addiction and other relevant management or health care issues.

• Provides Clinical Supervision for Master’s level social workers.

• May direct and monitor additional smaller programs to adhere to contract guidelines and acceptable clinical standards of practice.

• Accepts and performs other related duties and responsibilities as required.

REQUIRED QUALIFICATIONS:

• Master’s degree in Social Work (MSW) or a Master’s degree in a comparable or related social services discipline.

• Five years of experience working directly with disabled adults and/or the frail elderly.

• Demonstrates ability to supervise intensive care management staff who work with adults with disabilities at risk of hospitalization, which present with complex medical conditions, mental health diagnoses, substance abuse, as well as physical rehabilitation needs.

• Demonstrates understanding of behavioral health services and behavioral health interventions.

• Proven previous training of staff as it relates to the population served through CLF.

• Demonstrates understanding of the health and psychosocial needs of this population.

• Displays knowledge of community resources available for this population.

• Demonstrates ability and willingness to take on program development projects.

• Proven ability and willingness to provide leadership amongst peers and mentor junior staff.

Compensation

• Range: $57.25 - $60.00/hourly
