Job summary

Palms Home Care in Lakeland, FL seeks a Clinical Supervisor to lead clinical staff and ensure delivery of high-quality patient care in the home health setting.

You will review patient information, assign clinicians, conduct performance evaluations, and coordinate training while collaborating with interdisciplinary teams to implement care plans and maintain compliance with best practices and regulatory standards.

Qualifications

• RN licensure in good standing.

Job description

We are seeking a dedicated and experienced Clinical Supervisor to join our Palms Home Care team. The ideal candidate will provide leadership, guidance, and support to clinical staff to ensure the delivery of high-quality patient care.

Responsibilities

• Reviews available patient information related to case, including disciplines required, to determine home care needs. Assigns appropriate clinicians to case.
• Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
• Supervise and mentor clinical staff, including therapists, counselors, and other healthcare professionals.
• Conduct regular performance evaluations and provide constructive feedback to staff.
• Facilitate clinical training and professional development opportunities.
• Collaborate with interdisciplinary teams to develop and implement patient care plans.
• Monitor patient outcomes and ensure adherence to best practices and clinical guidelines.
• Assist in resolving clinical issues and addressing staff concerns.
• Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals.
• Performs other duties and activities as delegated by the Clinical Director.

Qualifications
