Job summary

Palms Home Care seeks a dedicated Clinical Supervisor to lead our clinical team and ensure high-quality patient care. You will supervise staff, mentor clinicians, and collaborate with interdisciplinary teams to implement care plans.

The ideal candidate holds an RN license with supervisory experience in home health, familiarity with OASIS and EMR systems (Kinnser), and a commitment to patient confidentiality and regulatory standards. This position requires a background check.

Qualifications

• RN licensure in good standing.
• At least one year of management or supervisory experience in home health.
• Familiarity with OASIS, care documentation requirements, and interdisciplinary coordination.
• Experience using electronic medical record (EMR) systems such as Kinnser.
• Knowledge of clinical best practices and regulatory standards.
• Commitment to maintaining patient confidentiality and ethical standards.

Responsibilities

• Reviews patient information to determine home care needs and assigns clinicians.
• Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
• Supervise and mentor clinical staff, including therapists and other healthcare professionals.
• Conduct regular performance evaluations and provide feedback to staff.
• Facilitate clinical training and professional development opportunities.
• Collaborate with interdisciplinary teams to develop and implement patient care plans.
• Monitor patient outcomes and ensure adherence to best practices and clinical guidelines.
• Assist in resolving clinical issues and addressing staff concerns.
• Assist in screening and interviewing new personnel and make recommendations for employment.
• Perform duties as delegated by the Clinical Director.

Responsibilities

• Reviews available patient information related to case, including disciplines required, to determine home care needs. Assigns appropriate clinicians to case.
• Reviews patient’s clinical diagnosis, medications, procedures and clinical course.
• Supervise and mentor clinical staff, including therapists, counselors, and other healthcare professionals.
• Conduct regular performance evaluations and provide constructive feedback to staff.
• Facilitate clinical training and professional development opportunities.
• Collaborate with interdisciplinary teams to develop and implement patient care plans.
• Monitor patient outcomes and ensure adherence to best practices and clinical guidelines.
• Assist in resolving clinical issues and addressing staff concerns.
• Assists in the screening and interviewing process of new organization personnel and makes recommendations for employment of individuals.
• Performs other duties and activities as delegated by the Clinical Director.
