Required – Experience & Education

Bachelor of Science (BS) degree or Doctor of Pharmacy (PharmD) degree
1 year of pharmacy experience
English speaking, reading, and writing
Eligible for licensure by Texas State Board of Pharmacy (TSBP) upon hire; Texas license within 30 days
ACPE‑IV Sterile Compounding (IV) certification upon hire
Basic Life Support (BLS) certification upon hire
Cardiac Life Support training within 180 days for inpatient positions (accredited by American Heart Association or American Red Cross)

Job Duties

Perform job responsibilities in accordance with organizational and departmental policies, procedures, performance improvement objectives and environmental/infection control standards as well as state and federal laws and regulations.
Perform all computer order entry accurately, thoroughly and timely; review/update medication profiles and manage patient charges.
Review medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety; seek clarification if necessary and document interventions.
Consistently multi‑task, problem‑solve independently and effectively with minimal supervision; exercise judgment and cross‑train in multiple areas.
Enforce the formulary system and follow departmental policy when non‑formulary product acquisition is necessary; participate in cost‑effective use and selection of pharmaceuticals.
Communicate drug information to physicians, nurses, etc. in a courteous and professional manner; assist in patient/family education and counseling regarding drug therapy and side effects; document clinical activities and demonstrate cost savings.
Manage technicians and other supportive personnel; delegate tasks and validate accuracy of medications intended for distribution; audit and sign out all controlled substances in accordance with state/federal guidelines and policies.
Actively participate in educational programs, orientation, and training; take on additional projects as needed.
Support P&T initiatives by monitoring target drugs for protocol discontinuation, substitution, IV to PO, etc.; collect applicable data and communicate improvement opportunities.
Perform therapeutic monitoring of specified drug therapies and recommend dosage adjustments as needed based on renal function evaluation and pharmacokinetic calculations.
Duties may include one or more of the following core functions: a) Direct patient interaction; b) Direct interaction with research participants; c) Regularly modify or release patient records; d) Regularly modify or release research records.
Perform other duties as assigned.
