Job summary

Wheeling Hospital in West Virginia is seeking a Clinical Social Worker to join its behavioral health team. The role collaborates with Primary Care Providers and a collaborating psychiatrist to deliver comprehensive assessment, treatment planning, and care management.

You will carry a caseload, perform psychosocial assessments, provide psychoeducation, and connect patients and families with appropriate resources.

Qualifications

• Master's Degree in Social Work is required.
• Two (2) years of direct clinical experience with behavioral health.
• Current license by state board where services will be provided.

Responsibilities

• Provides psychosocial assessments of patients and families to identify emotional, social, and enviromental strengths and problems.
• Collaborates with the multidisciplinary team to provide comprehensive mental health support for designated caseload.
• Monitors patient progress using validated scales and clinical judgement; adjusts care plans as needed.
• Provides brief interventions, psychoeducation, and care management for patients and families.
• Develops individualized care plans and arranges resources in collaboration with the care team.
• Implements psychosocial treatment plans including crisis intervention and family therapies.
• Works with providers on diagnosis, treatment, and management of mental health/substance use issues.
• Documents assessments and interventions according to guidelines.

Minimum Qualifications

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

• Master's Degree in Social Work or related field
• Must possess current license as required by state board where services will be provided:

MD: Licensed Master Social Worker (LMSW), Licensed Certified Social Worker (LCSW) or Licensed Certified Social Worker –Clinical (LCSW-C) through the Maryland Board of Social Work

OH: Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW) through the Ohio Board of Social Work

PA: Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) Pennsylvania Board of Social Workers, Marriage and Family Therapists and Professional Counselors

WV: Licensed Graduate Social Worker (LGSW), Licensed Certified Social Worker (LCSW), or Licensed Independent Clinical Social Worker (LICSW) through the West Virginia Board of Social Work

Experience

CORE DUTIES AND RESPONSIBILITIES:

• Provides psychosocial assessments of patients and families to identify emotional, social, and environments strengths and problems related to their diagnosis, illness, treatment and/or life situation.
• Collaborates with all members of the multidisciplinary team to provide comprehensive mental health support for designated caseload.
• Monitors the patient’s progress by use of validated scales and clinical judgement; intervening as necessary and appropriate, to ensure that the plan of care and services provided are patient focused, high quality, efficient, and cost effective.
• Provides appropriate brief interventions, psychoeducation, direct care management to patients and families around issues related to the patient’s diagnosis, illness, treatment and/or life situation.
• Meets with the patient and/or family to assess needs and develop an individualized care plan and to arrange resources to meet the individual’s needs in collaboration with the patient’s care team.
• Formulates, develops, and implements a comprehensive psychosocial treatment plan utilizing appropriate clinical social work treatments and interventions. Interventions may include crisis intervention, brief individual, marital, family and/or group therapies as well as grief and bereavement work.
• Works with the multidisciplinary team to assist with screening, identification, diagnosis, management and treatment of victims of abuse, neglect, and domestic violence, rape, etc. Provides consultative services to health care team members within scope of care definitions, as needed. Maintain a working knowledge of relevant medical/legal issues that impact on patient care, e.g., advance directives, child and elder abuse
• Works closely with Primary Care Provider and Collaborating Psychiatrist on identified patients to diagnose, treat and manages mental health and/or substance abuse problems in patients and family members.
• Documents assessment, plan, interactions, and interventions according to departmental, hospital and/or health system guidelines and standards.
• Knowledge of hospital, medical center and/or health system resources to access and provide for patient care needs.
• Maintains a working knowledge of therapeutic techniques and interventions as well as awareness of resources available.
• Keeps current with social work and health care developments and seeks to increase further enhancements of job related knowledge.
• Provides teaching and training for students, staff, and faculty from social work and other disciplines. Contribute to the academic mission of the medical center and health system by participating in academic activity, such as committees, conferences, publications, teaching and research.
• Exhibits professional behavior on a consistent basis.
• Attends and participates in staff, committee, department, and other administrative meetings.
