Job summary

Sunrise Hospital in Las Vegas, NV is seeking a Social Worker to enhance problem solving and coping for patients and families throughout the hospital setting. The role provides emotional support, psychosocial assessment, supportive counseling, crisis intervention, group therapy and referrals to community resources.

The Social Worker completes psychosocial assessments within two working days, collaborates with treatment staff, and coordinates post-hospitalization care with community resources.

Qualifications

  • Master’s degree in social work required.
  • 1+ years of experience or MSW internship required.
  • Currently licensed in Nevada as a Registered Master of Social Work.
  • Licensed Clinical Social Worker (LCSW) preferred.
  • Certification in Social Work preferred.

Responsibilities

  • Complete psychosocial assessments on patients referred within two working days.
  • Demonstrate a basic understanding of psychosocial issues related to hospitalization for patients of all age populations.
  • Participate in interdisciplinary meetings and collaborate with treatment staff to coordinate care of a patient.
  • Participate in formulating policies and procedures to facilitate integration of clinical social work services with other disciplines.
  • Demonstrate knowledge of social service policies and procedures and provide staff with in‑service education.
  • Develop relationships with community agencies and serve as liaison to coordinate continuity of care for patients.
  • Plan and coordinate with staff of community facilities, agencies, health department and other resources to meet patient’s post‑hospitalization needs.
  • Provide community and professional staff training regarding patient psychosocial needs and coping with illness, grief and loss.
  • Provide evaluation and referral services for patients who are alcohol or drug dependent.

Job Summary
