Job summary

Responsibilities

• Provides clinical services to address emotional, social, and physical well-being.
• Screens and triages patients to identify needs and plan care.
• Conducts psychosocial assessments and delivers services as indicated.
• Collaborates with multidisciplinary team for continuity of care planning.
• Documents in medical records and communicates with the care team.
• Participates in CQI efforts and may supervise staff or students.
