You will work with a multi-disciplinary healthcare team to conduct assessments, develop care plans, coordinate services, and advocate for clients and families, promoting well‑being and high-quality care.

Qualifikationen

• Bachelor's or Master’s Degree in Social Work (MSW or BSW) from an accredited institution.
• Current state licensure as a Licensed Social Worker (LSW) or registration as an RSW depending on state.
• Minimum of one year of social work experience preferred.
• Complies with all relevant professional standards of practice.
• Current CPR if applicable.
• TB questionnaire/PPD or chest x-ray if applicable.
• Current Health certificate per contract or state regulation.
• Must meet all federal, state and local requirements.
• Must be at least 18 years of age.
