Job summary

Harbor Behavioral Health in Hopkinsville, KY is seeking a Clinical Social Worker (CSW) to join a newly opened, growing program. This full-time position reports to the Clinical Director and requires CSW licensure, with mentorship and structured supervision provided.

You will conduct intake and assessments, develop treatment plans, document outcomes, and collaborate with peers, families, and residential staff to coordinate care.

Qualifications

• Master's Degree in Social Work (MSW) required.
• Active CSW license required.
• Recent graduates encouraged to apply; practicum acceptable.
• Strong communication and teamwork skills required.

Responsibilities

• Complete comprehensive intake assessments and clinical interviews.
• Develop individualized treatment plans meeting medical necessity and payer requirements.
• Provide clinical oversight to Peer Support Specialists and review documentation.
• Maintain timely, accurate EMR documentation and required reporting.
• Collaborate with families, guardians, providers, and residential staff.
• Respond to clinical emergencies per Harbor Behavioral Health policies.
• Participate in treatment team meetings and discharge planning.
