Job summary

Qualifications

• LMSW/LCSW licensure endorsed by the state of Maryland.
• Master of Social Work degree.
• 3–5 years hospital experience preferred.

Responsibilities

• Conduct comprehensive psychosocial assessments and develop collaborative discharge plans with interdisciplinary teams.
• Provide crisis intervention, counseling, and emotional support to patients and families while facilitating support groups and educational sessions.
• Coordinate admissions and ongoing case management activities, including screening, transportation, authorization procurement, and appeals management.
• Participate in patient rounds, care conferences, and interdisciplinary meetings to support treatment planning and continuity of care.
• Maintain accurate documentation of assessments, interventions, plans, and outcomes per hospital policies and reporting requirements.
• Contribute to program development, performance improvement initiatives, and collaboration with stakeholders.
